This is a combination chemotherapy regimen consisting of three agents: cabazitaxel, mitoxantrone, and prednisone. Cabazitaxel is a second-generation taxane that acts as a microtubule inhibitor, disrupting cell division by stabilizing microtubules and inducing tumor cell death. It has low affinity for the P-glycoprotein efflux pump, allowing it to retain activity in tumors resistant to other taxanes such as docetaxel. Mitoxantrone is an anthracenedione antineoplastic agent that intercalates into DNA and inhibits topoisomerase II, leading to DNA strand breaks and apoptosis. Prednisone is a synthetic glucocorticoid with anti-inflammatory and immunosuppressive properties; in this context, it also helps manage side effects of chemotherapy and may have direct antitumor effects in prostate cancer. This combination has been studied primarily for metastatic castration-resistant prostate cancer (mCRPC), including both chemotherapy-naïve patients and those previously treated with docetaxel-based regimens. Clinical trials have demonstrated that the approved single-agent doses of cabazitaxel and mitoxantrone can be safely combined with prednisone, resulting in durable tumor responses[6][7][9]. The regimen leverages non-overlapping mechanisms of action from each drug component.
