Overview

This is a four-drug combination regimen consisting of carboplatin (a platinum-based chemotherapy), paclitaxel (a microtubule inhibitor chemotherapy), durvalumab (a PD-L1 immune checkpoint inhibitor monoclonal antibody), and tremelimumab (a CTLA-4 immune checkpoint inhibitor monoclonal antibody). The combination is under investigation for the treatment of various advanced solid tumors, including non-small cell lung cancer (NSCLC) and ovarian cancer. The rationale for this regimen is to combine cytotoxic chemotherapy with dual immune checkpoint blockade, aiming to enhance antitumor efficacy by both direct tumor cell killing and potentiation of the host immune response against cancer cells[2][3][9]. Durvalumab blocks PD-L1, preventing it from binding to PD-1 and CD80, thereby enhancing T-cell activity. Tremelimumab blocks CTLA-4, further promoting T-cell activation. Carboplatin induces DNA crosslinking leading to apoptosis in rapidly dividing cells, while paclitaxel stabilizes microtubules inhibiting mitosis.
