Overview

Caspofungin + clindamycin is a combination therapy used primarily as a salvage or alternative treatment for Pneumocystis jirovecii pneumonia (PCP), especially in patients who are intolerant of or have failed standard therapy with trimethoprim/sulfamethoxazole (TMP/SMZ). Caspofungin is an echinocandin antifungal that inhibits the synthesis of β-(1,3)-D-glucan, an essential component of the fungal cell wall, particularly effective against the cyst form of Pneumocystis jirovecii. Clindamycin is a lincosamide antibiotic that inhibits protein synthesis by binding to the 50S ribosomal subunit; its mechanism against P. jirovecii is not fully understood but may involve inhibition of protozoal and bacterial protein synthesis. This combination has been reported to be beneficial in case reports and small series for moderate-to-severe PCP when first-line therapies are contraindicated or ineffective.
