Overview

Caspofungin + trimethoprim + sulfamethoxazole is a combination therapy used primarily for the treatment of Pneumocystis pneumonia (PCP). This combination works through complementary mechanisms: caspofungin inhibits the cyst form of Pneumocystis jirovecii by targeting the fungal cell wall synthesis, while trimethoprim-sulfamethoxazole (TMP-SMX) inhibits two consecutive steps in bacterial folate synthesis, disrupting nucleic acid and protein production. Clinical studies have shown that this combination therapy provides better efficacy than TMP-SMX monotherapy, with higher positive response rates and lower mortality rates in both HIV-infected and non-HIV-infected patients with moderate-to-severe PCP.
