CD123 + NKG2D dual-target CAR-T (also known as 123NL CAR-T) is an investigational dual-targeted chimeric antigen receptor T-cell (CAR-T) therapy being developed by Tianjin First Central Hospital and Nankai University for the treatment of CD123-positive relapsed or refractory acute myeloid leukemia (AML). The CAR-T construct simultaneously targets CD123 (interleukin-3 receptor subunit alpha) and NKG2D ligands (NKG2DLs) expressed on AML cells, as well as on immunosuppressive cells in the tumor microenvironment, such as myeloid-derived suppressor cells (M-MDSCs) and M2 macrophages. This dual-targeting approach is designed to overcome antigen escape and the immunosuppressive tumor microenvironment. Additionally, the construct incorporates the RQR8 safety switch (a compact marker/suicide gene containing CD34 and CD20 epitopes), allowing for the selective elimination of the CAR-T cells via rituximab administration if severe toxicity occurs.
