This combination immunotherapy regimen is being evaluated in a Phase 2 clinical trial at Ruijin Hospital for the treatment of high-risk relapsed or refractory large B-cell lymphoma (r/r LBCL). The approach combines CD19-targeted chimeric antigen receptor (CAR) T-cell therapy with glofitamab, a CD20×CD3 bispecific T-cell engaging antibody. The treatment protocol involves a sequence of obinutuzumab pretreatment to deplete peripheral B cells and mitigate cytokine release syndrome (CRS), followed by glofitamab step-up dosing as bridging therapy, lymphodepletion with fludarabine and cyclophosphamide, and finally the infusion of CD19-targeted CAR T-cells. Patients who do not achieve a complete response may receive additional glofitamab consolidation cycles. This dual-antigen targeting strategy (CD19 and CD20) aims to overcome antigen escape and improve the durability of responses in aggressive B-cell malignancies.
