Overview

CAR19-T-cell therapy refers to a class of adoptive cellular immunotherapies where a patient's T cells are genetically engineered to express a chimeric antigen receptor (CAR) targeting the CD19 antigen, which is widely expressed on B-cell lineage cells. This therapy is primarily used to treat refractory or relapsed B-cell malignancies, including Diffuse Large B-cell Lymphoma (DLBCL), Follicular Lymphoma (FL), and Acute Lymphoblastic Leukemia (ALL). The process involves leukapheresis, ex vivo T-cell modification and expansion, and re-infusion into the patient following lymphodepleting chemotherapy. While highly effective, it is associated with significant toxicities such as cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), and prolonged cytopenias that increase the risk of opportunistic infections.
