Overview

CD19-targeting CAR-T refers to a class of chimeric antigen receptor (CAR) T-cell therapies engineered to target the CD19 antigen, a surface protein expressed on B-lineage cells. These therapies involve the genetic modification of T cells to express a CAR construct, which typically includes an extracellular anti-CD19 single-chain variable fragment (scFv) for antigen recognition, a transmembrane domain, and intracellular signaling domains (such as CD3-zeta and costimulatory domains like 4-1BB or CD28). Upon re-infusion into the patient, these engineered cells recognize and eliminate CD19-positive cells, including both malignant and normal B cells. This modality has been successfully commercialized for various B-cell malignancies, including acute lymphoblastic leukemia (B-ALL) and several types of non-Hodgkin lymphoma. Recent clinical research has also demonstrated significant potential for CD19-targeting CAR-T cells in treating severe B-cell-mediated autoimmune diseases, such as systemic lupus erythematosus (SLE), systemic sclerosis, and inflammatory myositis. Emerging approaches also include in vivo CAR-T generation using viral vectors or lipid nanoparticle (LNP) delivery systems.
