Overview

CD20-specific CAR T cells are autologous or allogeneic T lymphocytes genetically modified to express a chimeric antigen receptor (CAR) targeting the CD20 antigen found on the surface of B cells. The CAR is a synthetic protein that consists of an extracellular domain derived from an anti-CD20 antibody fragment, a transmembrane domain, and intracellular activation domains such as CD3 zeta and co-stimulatory signals (like CD28 or 4-1BB). Upon binding to CD20, these cells become activated, proliferate, and induce cytotoxicity against CD20-positive malignant cells. The primary indication is relapsed or refractory B-cell non-Hodgkin lymphoma (NHL), and clinical studies have also explored activity in other B-cell malignancies. CD20-specific CAR T cells are in clinical trials and are not yet FDA approved. Preclinical and early clinical data suggest these therapies may be effective even when CD20 expression is low or after failure of anti-CD20 monoclonal antibody therapies[1][3][4][5].
