CD3 x CD20 bispecific antibodies are a class of immunotherapy agents designed to treat B-cell malignancies by simultaneously binding to CD3 on T cells and CD20 on B cells. This dual binding facilitates the formation of an immunological synapse, leading to T-cell activation and the subsequent directed lysis of the target B cells. Several agents in this class, such as epcoritamab, glofitamab, and mosunetuzumab, have received regulatory approval for indications including diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma. They are characterized by their ability to induce deep and durable responses in relapsed or refractory settings, though they are associated with specific toxicities like cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS). These agents offer an off-the-shelf alternative to CAR-T cell therapies.
