Overview

This is a combination therapy used primarily in allogeneic hematopoietic stem cell transplantation (HSCT), especially in haploidentical or unrelated donor settings. The regimen consists of a CD34-selected hematopoietic stem cell graft (enriched for CD34+ progenitor cells) combined with anti-thymocyte globulin (ATG), an immunosuppressive polyclonal antibody preparation targeting human T lymphocytes and other immune cells. The main purpose of this combination is to facilitate engraftment and reduce the risk of both acute and chronic graft-versus-host disease (GVHD). - **Mechanism:** - The CD34-selected graft provides hematopoietic progenitor cells while depleting T cells, reducing the risk of GVHD. - Anti-thymocyte globulin depletes recipient and donor T lymphocytes through multiple mechanisms including complement-mediated lysis, apoptosis induction, and functional inhibition, further lowering GVHD risk but also delaying immune reconstitution[2][4][1]. - **Indications:** Used as part of conditioning regimens for patients undergoing allogeneic HSCT for diseases such as acute leukemia or myelodysplastic syndrome[1][4].
