Overview

Chlormethiazole + haloperidol is a combination therapy consisting of two distinct drugs used together, primarily in the management of delirium tremens and severe alcohol withdrawal. Chlormethiazole is a sedative and hypnotic agent that acts as a central nervous system depressant, mainly by potentiating GABAergic neurotransmission, leading to its calming effects[4][1]. Haloperidol is a typical (first-generation) antipsychotic that exerts its primary effect through antagonism of dopamine D2 receptors in the brain, reducing psychotic symptoms such as hallucinations and agitation[2][7]. The rationale for combining these agents is to address both the neurovegetative (autonomic) disturbances with chlormethiazole and the psychotic features with haloperidol during acute alcohol withdrawal syndromes like delirium tremens[1].
