This combination regimen is a sequential chemoimmunotherapy and radioimmunotherapy protocol developed by the University of Pittsburgh for the treatment of previously untreated follicular lymphoma. The regimen consists of an induction phase with three cycles of CHOP-R (cyclophosphamide, doxorubicin, vincristine, prednisone, and rituximab), followed by consolidation with the radiopharmaceutical yttrium-90-ibritumomab tiuxetan (Zevalin), and a maintenance phase of extended rituximab therapy. The strategy aims to maximize complete response rates by combining the systemic cytotoxic effects of chemotherapy with the targeted antibody-mediated depletion and localized beta-radiation delivery of radioimmunotherapy, while minimizing long-term toxicity by limiting the number of chemotherapy cycles. The primary biological target is the CD20 antigen expressed on the surface of B-lymphocytes.
