Ciprofloxacin + meropenem is a combination of two broad-spectrum antibacterial agents used together to enhance bactericidal activity and suppress resistance, particularly in severe or resistant infections. Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, thereby blocking DNA replication and transcription. Meropenem is a carbapenem beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). The combination has demonstrated synergistic effects against difficult-to-treat pathogens such as Pseudomonas aeruginosa—including hypermutable strains from cystic fibrosis patients—and carbapenem-resistant Acinetobacter baumannii. Mechanistically, the synergy arises from increased outer membrane permeability, inhibition of efflux pumps, disruption of metabolic pathways, and enhanced reactive oxygen species generation in bacteria. This combination is considered for use in critically ill patients or those with augmented renal clearance where monotherapy may be insufficient[1][2][3][4][5].
