This is a combination of three antibiotics—clindamycin, flucloxacillin, and rifampicin—used together primarily for the treatment of serious infections caused by susceptible bacteria, especially staphylococcal infections such as prosthetic joint infection (PJI). - Clindamycin is a lincosamide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. It is effective against anaerobic bacteria and some Gram-positive cocci[2][6]. - Flucloxacillin is a β-lactamase-resistant penicillin (a penam) that acts by inhibiting bacterial cell wall synthesis, particularly effective against Staphylococcus aureus[5]. - Rifampicin (rifampin) inhibits bacterial DNA-dependent RNA polymerase, thereby suppressing RNA synthesis and subsequent protein production. It has broad-spectrum activity but is most commonly used in combination regimens to prevent resistance development. This triple therapy may be considered in complex or resistant staphylococcal infections where monotherapy or dual therapy may not suffice. The combination leverages different mechanisms of action to maximize bactericidal effect and minimize resistance risk[1][4].
