clindamycin + piperacillin + tazobactam + vancomycin

This is a combination antibiotic regimen used primarily for the treatment of necrotizing soft tissue infections (NSTIs). The combination provides broad-spectrum coverage against gram-positive bacteria (including methicillin-resistant Staphylococcus aureus), gram-negative bacteria, and anaerobes. Clindamycin inhibits bacterial protein synthesis and suppresses toxin production, particularly in group A Streptococcus infections. Piperacillin is a semisynthetic, broad-spectrum ureidopenicillin antibiotic that is combined with tazobactam (a beta-lactamase inhibitor) to overcome resistance. Vancomycin is a glycopeptide antibiotic effective against gram-positive bacteria. However, this combination has been associated with increased risk of acute kidney injury (AKI) and Clostridioides difficile infection.
