Overview

Cyclophosphamide + dacarbazine + vincristine (commonly referred to as the CVD regimen) is a combination chemotherapy protocol consisting of three cytotoxic agents. Cyclophosphamide is an alkylating agent that interferes with DNA replication and RNA transcription, leading to cell death. Dacarbazine is also an alkylating agent that works by methylating guanine in DNA, resulting in inhibition of DNA synthesis and function. Vincristine is a vinca alkaloid that inhibits microtubule formation in mitotic spindle fibers, arresting cell division at metaphase. The combination exploits different mechanisms to maximize antitumor activity and minimize resistance. The CVD regimen has been most notably used for the palliative treatment of malignant pheochromocytoma and paraganglioma, where it can induce partial tumor responses or biochemical improvements in a significant proportion of patients[1][2][3][5][8]. It may also be considered for other rare neuroendocrine tumors but is not curative[1]. The drugs are administered intravenously on specific days within each cycle; typical regimens involve repeated cycles every 3–4 weeks[6].
