Overview

R-CHOP + rituximab is a chemoimmunotherapy regimen consisting of the anti-CD20 monoclonal antibody rituximab combined with the CHOP chemotherapy backbone, which includes cyclophosphamide, doxorubicin (hydroxydaunorubicin), vincristine (Oncovin), and prednisone. Rituximab targets the CD20 antigen expressed on the surface of B-cells, inducing cell death through antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). The chemotherapy components provide broad cytotoxic activity: cyclophosphamide acts as a DNA alkylating agent; doxorubicin intercalates DNA and inhibits topoisomerase II; vincristine disrupts microtubule formation during mitosis; and prednisone exerts lymphocytolytic effects. This specific protocol is being evaluated in clinical trials, such as those led by Ruijin Hospital, to determine if a reduced-chemotherapy schedule (e.g., 4 cycles of R-CHOP) supplemented with additional rituximab cycles can maintain efficacy while reducing long-term toxicity in patients with newly diagnosed, low-risk, non-bulky diffuse large B-cell lymphoma (DLBCL).
