Overview

Cyclopropyl-methoxycarbonyl metomidate (CPMM) is an experimental, ultra-short-acting intravenous anesthetic and sedative agent. It is a "soft-drug" analogue of metomidate (the methyl ester of etomidate), designed to undergo rapid hydrolysis by non-specific tissue esterases into an inactive carboxylic acid metabolite. This metabolic design aims to combine the favorable hemodynamic stability characteristic of etomidate and metomidate with a rapid recovery profile, while specifically avoiding the sustained adrenocortical suppression (inhibition of 11β-hydroxylase) that typically limits the use of etomidate for continuous infusion. CPMM acts as a positive allosteric modulator of the GABAA receptor, facilitating inhibitory neurotransmission in the central nervous system. It has been investigated in clinical trials for the induction and maintenance of general anesthesia and for procedural sedation.
