dexamethasone + rituximab + romiplostim

This is a combination therapy consisting of three agents—dexamethasone, rituximab, and romiplostim—used primarily in the treatment of newly diagnosed immune thrombocytopenia (ITP). Dexamethasone is a synthetic glucocorticoid with potent anti-inflammatory and immunosuppressive properties. Rituximab is a monoclonal antibody targeting CD20 on B lymphocytes, leading to B cell depletion. Romiplostim is a thrombopoietin receptor agonist that stimulates platelet production by activating the c-Mpl (thrombopoietin) receptor on megakaryocyte precursors. This combination aims to address multiple pathogenic mechanisms in ITP by suppressing autoantibody production (rituximab), reducing immune-mediated platelet destruction (dexamethasone), and enhancing platelet production (romiplostim). Pilot studies have shown high response rates and good tolerability for this regimen in adults with newly diagnosed ITP[1][2][6].
