Drug intelligence / Profile preview
This is a combination of three different vasoactive medications commonly used in critical care settings, particularly for patients with shock. Each component has distinct pharmacological properties and mechanisms of action that, when combined, can provide comprehensive cardiovascular support.
Dobutamine is a synthetic catecholamine that primarily stimulates β1-adrenergic receptors, with some activity at β2 and α1 receptors. It's a potent inotropic agent that increases cardiac contractility and cardiac output with relatively minimal chronotropic effects (heart rate increase). At lower doses (≤5 μg/kg/min), dobutamine typically causes mild vasodilation, while at higher doses (>10-15 μg/kg/min), α1-mediated vasoconstriction becomes more prominent.
Dobutamine is commonly used for:
The typical dosing range is 2.5-20 μg/kg/min, with most inotropic effects seen at 0-10 μg/kg/min.
Dopamine is an endogenous catecholamine that acts as a precursor to noradrenaline. Its effects are dose-dependent:
Dopamine is used for:
Noradrenaline is a potent vasoconstrictor that primarily stimulates α1-adrenergic receptors with some β1 activity. It increases systemic vascular resistance and blood pressure while having less pronounced effects on heart rate compared to other catecholamines.
Noradrenaline is typically used for:
The combination of these three agents is not a fixed-dose formulation but rather represents a therapeutic strategy used in critical care. Each medication can be titrated independently to achieve desired hemodynamic effects:
1. Dobutamine provides inotropic support (increased cardiac contractility) 2. Dopamine offers both inotropic effects and potential renal perfusion benefits 3. Noradrenaline provides strong vasopressor support
This combination might be used in complex shock states where a single agent is insufficient to maintain adequate hemodynamics. For example, in septic shock with myocardial depression, noradrenaline might be used to maintain blood pressure while dobutamine is added to improve cardiac output.
Some studies have examined specific combinations, such as noradrenaline plus dobutamine versus noradrenaline plus epinephrine for cardiovascular support in septic shock.
The deliberate combination of dopamine and dobutamine at moderate doses (7.5 μg/kg/min each) has been shown to improve hemodynamics while limiting side effects compared to either agent alone at higher doses (15 μg/kg/min).
