Dobutamine + levosimendan is a combination of two intravenous inotropic agents used primarily in the management of acute decompensated heart failure (ADHF) and cardiogenic shock, particularly when patients are refractory to single-agent therapy. Dobutamine is a synthetic catecholamine that acts as a beta-1 adrenergic receptor agonist, increasing cardiac contractility and output. Levosimendan is a calcium sensitizer and potassium ATP-channel opener that enhances myocardial contractility without significantly increasing myocardial oxygen consumption, while also providing vasodilatory effects. The combination has been studied for its potential to improve hemodynamic parameters and clinical outcomes in patients who do not adequately respond to dobutamine alone. Clinical studies suggest that adding levosimendan to ongoing dobutamine therapy may increase the likelihood of achieving target hemodynamic goals (such as superior vena cava oxygen saturation), facilitate successful weaning from inotropes, and reduce major adverse cardiovascular events compared with dobutamine monotherapy[1][2][6]. This strategy may be particularly beneficial for patients with persistent low cardiac output despite standard inotropic support.
