Overview

**Duvelisib + rituximab** is a combination therapy being investigated primarily for hematologic malignancies such as chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), and non-Hodgkin lymphoma (NHL). Duvelisib is an oral small molecule dual inhibitor of phosphoinositide 3-kinase delta (PI3Kδ) and gamma (PI3Kγ) isoforms, which are pivotal in the signaling pathways regulating the proliferation, survival, and trafficking of malignant lymphocytes. Inhibition of these isoforms suppresses cellular proliferation and disrupts the tumor microenvironment. Rituximab is a chimeric monoclonal antibody targeting the CD20 antigen on B cells, leading primarily to B cell depletion via antibody-dependent cellular cytotoxicity, complement-dependent cytotoxicity, and direct induction of apoptosis. This combination is being evaluated in clinical trials for improved efficacy over single-agent therapies in relapsed/refractory and previously untreated hematologic malignancies, with early-phase studies showing promising response rates[1][3][5][7][9].
