ExoFITC-miR-1179 is an engineered exosome-based therapeutic candidate designed for the lung-targeted delivery of microRNA-1179 (miR-1179) to treat acute respiratory distress syndrome (ARDS). The exosomes are decorated with the C67-29 peptide to facilitate specific uptake by lung epithelial cells. Once delivered via nebulized inhalation, the miR-1179 cargo binds to the 3'UTR of High Mobility Group Box 1 (HMGB1) mRNA, suppressing its expression. HMGB1 is a key pro-inflammatory mediator; its downregulation inhibits the NF-κB pathway and reduces the production of inflammatory cytokines such as TNF-α, IL-6, and IL-1β. Preclinical studies in murine models have demonstrated that this targeted delivery approach significantly alleviates alveolar damage and inflammatory infiltration compared to unmodified exosomes or free miR-1179.
