Overview

A combination therapy consisting of fedratinib, a small molecule JAK2 inhibitor, and nivolumab, a human monoclonal antibody targeting the PD-1 checkpoint receptor. This combination is being evaluated in the Phase II FRACTION trial, sponsored by the Institut für Klinische Krebsforschung IKF GmbH at Krankenhaus Nordwest, for the treatment of patients with primary or secondary myelofibrosis who have shown resistance or suboptimal response to prior JAK inhibitor therapy. Fedratinib works by inhibiting Janus kinase 2 (JAK2) and FMS-like tyrosine kinase 3 (FLT3), which are often overactive in myeloproliferative neoplasms, while nivolumab enhances the anti-tumor immune response by preventing the interaction between PD-1 and its ligands (PD-L1 and PD-L2). The rationale for the combination is that JAK inhibition may modulate the immune microenvironment, potentially enhancing the efficacy of PD-1 blockade in myelofibrosis.
