Overview

The fixed neoantigen tumor vaccine is an mRNA-based biologic vaccine developed by Ruijin Hospital in collaboration with Shanghai Xinpu BioTechnology for the treatment of pancreatic cancer. It is designed to target common driver mutations prevalent in pancreatic adenocarcinoma, specifically KRAS variants such as G12D, G12V, G12R, and G12C. The vaccine delivers mRNA encoding these specific neoantigens to stimulate the activation of antigen-specific CD4+ and CD8+ T cells, thereby generating a targeted immune response against residual tumor cells following surgery. In clinical protocols, this "off-the-shelf" fixed vaccine is typically administered as a priming phase (often 4 cycles) followed by a personalized neoantigen vaccine (XP-004) and is used in combination with the PD-1 inhibitor toripalimab as adjuvant therapy for chemotherapy-intolerant patients.
