Overview

This is a combination of three antibiotics—gentamicin, trimethoprim, and sulfamethoxazole—used together for their synergistic antibacterial effects. Gentamicin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Trimethoprim is a dihydrofolate reductase inhibitor that blocks folic acid synthesis in bacteria. Sulfamethoxazole is a sulfonamide antibiotic that inhibits dihydropteroate synthase, another enzyme in the folic acid pathway. When used together (trimethoprim-sulfamethoxazole as co-trimoxazole), they block sequential steps in bacterial folate synthesis, resulting in enhanced bactericidal activity. The addition of gentamicin provides further synergy against certain Gram-negative organisms such as Escherichia coli and Klebsiella pneumoniae[1][2][3][5]. This combination has been studied for use against enteric infections and other infections caused by susceptible bacteria, particularly when resistance or severe infection warrants multi-drug therapy[5]. It has veterinary applications and may be considered for severe or resistant human infections.
