This combination regimen is an investigational therapy being evaluated for the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL). It consists of **homoharringtonine (HHT)**, a natural cephalotaxine alkaloid that acts as a protein synthesis inhibitor; **lisaftoclax (APG-2575)**, a potent and selective B-cell lymphoma 2 (BCL-2) inhibitor; **rituximab**, an anti-CD20 monoclonal antibody; and **prednisone**, a corticosteroid. The mechanistic rationale for this combination lies in the ability of homoharringtonine to downregulate the anti-apoptotic protein MCL-1, which is a common resistance factor for BCL-2 inhibitors. By combining these agents, the regimen aims to overcome apoptotic resistance and enhance clinical efficacy in patients with aggressive B-cell malignancies. The regimen is currently being studied in a Phase Ib/II trial led by the First Affiliated Hospital of Zhejiang University.
