Hu1928-Hu20BB-Long is a novel bicistronic chimeric antigen receptor (CAR) T-cell therapy developed by the National Cancer Institute (NCI) for the treatment of advanced B-cell malignancies. This cell therapy is engineered to simultaneously target two distinct antigens, CD19 and CD20, which are frequently expressed on the surface of malignant B cells. The construct utilizes humanized single-chain variable fragments (scFvs)—Hu1928 for CD19 and Hu20BB for CD20—and incorporates a 4-1BB (CD137) costimulatory domain to enhance T-cell persistence and metabolic fitness. The "Long" designation refers to the specific spacer length used in the CAR construct. By targeting two antigens, Hu1928-Hu20BB-Long aims to mitigate the risk of tumor escape through antigen loss, a common mechanism of resistance observed with single-target CAR T-cell therapies. It is currently being evaluated in Phase I clinical trials for patients with relapsed or refractory B-cell lymphoma and chronic lymphocytic leukemia.
