The combination of ibrutinib, venetoclax, and obinutuzumab is a fixed-duration, chemotherapy-free regimen developed for the treatment of chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including both treatment-naïve and relapsed/refractory patients. This triplet therapy leverages three targeted agents with complementary mechanisms: - Ibrutinib is a Bruton's tyrosine kinase (BTK) inhibitor that disrupts B-cell receptor signaling. - Venetoclax is a selective BCL2 inhibitor that induces apoptosis in malignant B cells. - Obinutuzumab is an anti-CD20 monoclonal antibody that mediates direct cell death and enhances immune-mediated cytotoxicity against CD20-expressing B cells. Clinical trials have demonstrated deep remissions with high rates of undetectable minimal residual disease (uMRD), allowing for time-limited therapy without traditional cytotoxic chemotherapy. The combination has shown efficacy in both first-line and relapsed/refractory CLL/SLL settings[1][3][5][10]. It has also been studied for Richter transformation but responses were less durable[7].
