IFN-γ-iMSC-EVs (also known by the development code BRE-AD01) are extracellular vesicles (EVs) or exosomes derived from induced mesenchymal stem cells (iMSCs) that have been primed with interferon-gamma (IFN-γ). Developed by Brexogen, this cell-free therapeutic candidate is designed to treat inflammatory skin conditions, primarily atopic dermatitis. The IFN-γ priming enhances the immunomodulatory cargo of the EVs, allowing them to suppress Th2-mediated immune responses by downregulating the expression of receptors for IL-4, IL-13, and IL-31 (IL-4Rα, IL-13Rα1, and IL-31Rα) and inhibiting the downstream JAK/STAT signaling pathway (specifically JAK1, JAK2, and STAT6). Preclinical studies have shown that IFN-γ-iMSC-EVs effectively reduce pruritus, mast cell infiltration, and skin thickness while promoting skin barrier restoration through the upregulation of epidermal differentiation proteins like Keratin 1 and Keratin 10. The candidate is currently in Phase 1 clinical trials in the United States for the treatment of moderate-to-severe atopic dermatitis.
