INCB040093 + itacitinib is an investigational combination therapy for relapsed or refractory B-cell malignancies, including non-Hodgkin lymphoma and classical Hodgkin lymphoma. INCB040093 is a selective phosphatidylinositol 3-kinase delta (PI3Kδ) inhibitor that blocks PI3Kδ-mediated cell growth and induces apoptosis in B-cell lymphomas. Itacitinib (formerly known as INCB039110) is a selective Janus kinase 1 (JAK1) inhibitor with high selectivity over JAK2 and JAK3. The rationale for combining these agents is based on the involvement of both the PI3Kδ and JAK-STAT pathways in tumor cell proliferation and survival; their simultaneous inhibition may provide synergistic efficacy. Clinical studies have shown that this combination has activity across various B-cell lymphomas, with particularly promising results in classic Hodgkin lymphoma subtypes[2][5]. The combination also appears to reduce certain toxicities seen with single-agent PI3Kδ inhibitors, such as hepatotoxicity[6]. Both drugs are small molecules developed by Incyte.
