Indium-111 ibritumomab tiuxetan is a radiolabeled monoclonal antibody used as part of the Zevalin therapeutic regimen for non-Hodgkin's lymphoma. It consists of the murine IgG1 kappa monoclonal antibody (ibritumomab) conjugated to the chelator tiuxetan, which binds radioactive indium-111. The antibody targets CD20 antigen on B lymphocytes, allowing imaging of biodistribution prior to therapy with yttrium-90-labeled ibritumomab tiuxetan. Mechanistically, it binds CD20 on normal and malignant B cells; radiation from the attached isotope induces cell death in targeted and neighboring cells via beta emission. The antibody also mediates cytotoxicity through ADCC, CDC, and apoptosis[2][3][5]. Developed by IDEC Pharmaceuticals (now part of Biogen), it was approved for relapsed or refractory low-grade or follicular B-cell non-Hodgkin's lymphoma[5][8].
