Itacitinib + calcineurin inhibitor is an investigational combination therapy consisting of itacitinib, a selective Janus kinase 1 (JAK1) inhibitor, and a calcineurin inhibitor (CNI, such as tacrolimus or cyclosporine). Itacitinib suppresses the JAK-STAT signaling pathway involved in immune activation, while calcineurin inhibitors block the phosphatase activity of calcineurin, thereby inhibiting T-cell activation and cytokine production. This combination is being evaluated primarily for the prophylaxis and treatment of graft-versus-host disease (GVHD) in patients undergoing allogeneic hematopoietic cell transplantation, with the aim of improving immunosuppressive efficacy and safety profiles compared to standard regimens[5][1][3].
