itacitinib + gemcitabine + nab-paclitaxel is an investigational combination regimen consisting of itacitinib (INCB039110), a selective Janus kinase 1 (JAK1) inhibitor; gemcitabine, a nucleoside analog chemotherapy; and nab-paclitaxel, a nanoparticle albumin-bound formulation of paclitaxel. This combination is being studied primarily for the treatment of advanced solid tumors, including metastatic and locally advanced pancreatic cancer. Preclinical rationale centers on the role of cytokine-mediated JAK/STAT signaling in tumor growth and cachexia, which itacitinib targets, while gemcitabine and nab-paclitaxel act as cytotoxic agents. In phase Ib/II studies, this regimen showed an acceptable safety profile and clinical activity, with promising reductions in tumor size and disease control rates for pancreatic cancer patients[1][4][7].
