KIR3DL2-targeted CAR-T cell therapy is an investigational, preclinical-stage cellular immunotherapy designed to treat T-cell lymphomas (TCL). It utilizes chimeric antigen receptor (CAR) T cells engineered to recognize KIR3DL2 (CD158k), a killer cell immunoglobulin-like receptor that is highly and specifically expressed on malignant T cells in cutaneous T-cell lymphoma (CTCL)—including Sézary syndrome and mycosis fungoides—and peripheral T-cell lymphoma (PTCL). Because KIR3DL2 expression is minimal on healthy T and NK cells, this therapy aims to minimize on-target, off-tumor toxicity. Research groups, such as those at the Peter MacCallum Cancer Centre, are also developing allogeneic ("off-the-shelf") versions using CRISPR-HDR technology to insert the CAR gene into the TRAC locus while modifying the B2M/HLA-E loci to prevent graft-versus-host disease and rejection.
