Lang-chuang-ding (also referred to as Langchuangding or LCD) is a traditional Chinese medicine formula that has been clinically used for treating systemic lupus erythematosus (SLE). It consists of multiple herbs including Cimicifugae Rhizoma (Sheng Ma), Paeoniae Radix rubra (Red Peony), Rehmanniae Radix, and Trionycis Carapax, among others. The formula works primarily by inducing apoptosis in activated T cells through up-regulating CASP3 expression levels. Research has shown that LCD can suppress the proliferation of activated T cells and induce their apoptosis, which is beneficial for treating SLE, an autoimmune disorder where activated T cells play a key role in pathogenesis. Clinical studies indicate that Lang-chuang-ding is effective for SLE treatment with fewer side effects and good patient compliance compared to conventional treatments. It's often used as a complementary treatment alongside conventional medications like prednisone, potentially allowing for lower doses of steroids and thus reducing side effects. Network pharmacology studies have identified several active components in the formula, with research suggesting that it works through multiple targets and pathways relevant to immune regulation and inflammation control. ## Composition and Formulation Lang-chuang-ding typically contains: - Rehmanniae Radix (18g) - Ophiopogon (12g) - Ching-hao (9g) - Isatis leaf (30g) - Oldenlandia (30g) - Red Peony (12g) - Moutan (10g) - Lithospermum (30g) - Campsis (9g) - Cimicifugae Rhizoma (Sheng Ma) (9g) - Radix Glycyrrhizae (Gan Cao) (6g) The formula is traditionally prepared as a decoction in water and taken in two divided doses, morning and night. ## Mechanism of Action Research using network pharmacology and experimental verification has revealed that Lang-chuang-ding works through several mechanisms: 1. Inducing apoptosis in activated T cells by up-regulating CASP3 expression levels 2. Suppressing the proliferation of activated T cells 3. Anti-inflammatory and immunomodulatory effects 4. Potentially regulating multiple signaling pathways involved in immune function These mechanisms contribute to its effectiveness in treating SLE, a complex autoimmune disorder that affects multiple organ systems.
