R²-DHAP is an experimental chemoimmunotherapy combination regimen consisting of the immunomodulatory drug lenalidomide added to the standard R-DHAP salvage therapy (rituximab, dexamethasone, high-dose cytarabine, and cisplatin). It is being investigated for the treatment of relapsed or refractory aggressive B-cell non-Hodgkin lymphomas, including diffuse large B-cell lymphoma (DLBCL), grade III follicular lymphoma, blastoid variant mantle cell lymphoma, Burkitt lymphoma, and aggressive marginal zone lymphoma. The regimen combines the anti-CD20 monoclonal antibody rituximab with cytotoxic chemotherapy and the pleiotropic effects of lenalidomide, which enhances T-cell and NK-cell mediated antibody-dependent cellular cytotoxicity (ADCC) and has direct antiproliferative effects. The combination is designed to overcome resistance in patients who have failed prior lines of therapy by utilizing multiple mechanisms of action, including DNA damage, immune modulation, and targeted B-cell depletion.
