Lutetium Lu 177 phosphonate is a therapeutic radiopharmaceutical being developed for the treatment of malignant bone metastases. It consists of the beta-emitting radioisotope lutetium-177 (Lu-177) complexed with a phosphonate ligand, which acts as a bone-seeking vector. Phosphonates have a high affinity for hydroxyapatite, the primary mineral component of bone, and preferentially accumulate at sites of increased bone remodeling, such as those found in osteoblastic or mixed metastatic lesions. Once localized, the Lu-177 isotope delivers targeted ionizing radiation (beta particles) to the tumor cells and the surrounding bone microenvironment, inducing DNA damage and cell death while minimizing systemic exposure. This agent is being evaluated for its safety, dosimetry, and preliminary efficacy in patients with skeletal metastases from various primary cancers.
