The Ma-Spore ALL-Seq 2020 protocol is a risk-stratified combination chemotherapy regimen developed by the National University Hospital, Singapore, in collaboration with institutions in Malaysia. It is designed for the treatment of children, adolescents, and young adults (up to age 41) with newly diagnosed B-cell acute lymphoblastic leukemia (B-ALL). The protocol represents a precision medicine approach, utilizing advanced molecular profiling techniques such as RNA-Seq and IgH/TCR-Seq to refine risk assignment and tailor treatment intensity. The regimen consists of a backbone of conventional chemotherapy agents, including vincristine, corticosteroids (prednisolone, dexamethasone), asparaginase formulations (PEG-asparaginase, Erwinase), antimetabolites (methotrexate, cytarabine, mercaptopurine, thioguanine), alkylating agents (cyclophosphamide), and anthracyclines (doxorubicin). For specific genetic subtypes, such as BCR-ABL1-positive or BCR-ABL1-like ALL, the protocol incorporates targeted tyrosine kinase inhibitors like dasatinib or imatinib, and the anti-CD20 monoclonal antibody rituximab for relevant cases.
