The MAID regimen (Mesna, Doxorubicin, Ifosfamide, and Dacarbazine) is a combination chemotherapy protocol primarily used for treating various types of sarcomas. Mesna acts as a uroprotective agent to prevent hemorrhagic cystitis caused by ifosfamide. Doxorubicin is an anthracycline antibiotic with antineoplastic properties. Ifosfamide is an alkylating agent that damages DNA and prevents cancer cell replication. Dacarbazine (DTIC) is an alkylating agent that also interferes with DNA replication. This combination therapy has shown efficacy in treating advanced soft tissue sarcomas, particularly in cases where single-agent therapy might be insufficient. It is typically administered intravenously in a hospital setting over multiple days. Clinical trials have demonstrated meaningful response rates with the MAID regimen, including a 47% overall response rate (10% complete response) in a phase II trial with 105 patients with advanced sarcoma. For retroperitoneal sarcoma patients, clinical benefit included partial response in 37.5% and stable disease in 50%. It has also been used as a treatment option for metastatic or recurrent uterine leiomyosarcoma.
