MDI-0355 (also known as MOD-C) is a small-molecule inhibitor of poly(ADP-ribose) glycohydrolase (PARG) being developed by Alivexis for the treatment of various cancers. PARG is the primary enzyme responsible for the degradation of poly(ADP-ribose) (PAR) polymers, which are synthesized by PARP enzymes in response to DNA damage. By inhibiting PARG, MDI-0355 causes the toxic accumulation of PAR chains, which disrupts DNA repair mechanisms and replication fork stability, leading to replication stress and selective cancer cell death. The drug is designed for broad-spectrum oncology applications, both as a monotherapy and in combination with chemotherapy, radiation, or other targeted therapies. It is currently in the IND-enabling stage of development.
