Mesalamine + rifaximin is a fixed-dose combination of two small molecule drugs used primarily for gastrointestinal disorders such as inflammatory bowel disease (IBD) and diverticular disease. Mesalamine (also known as mesalazine) is an anti-inflammatory agent that acts locally in the colon to inhibit the production of pro-inflammatory mediators, particularly by blocking cyclooxygenase and lipoxygenase pathways. Rifaximin is a non-absorbable antibiotic that reduces bacterial load in the gut by inhibiting bacterial RNA synthesis through binding to the beta-subunit of bacterial DNA-dependent RNA polymerase. The combination leverages complementary mechanisms—mesalamine reduces inflammation while rifaximin targets gut microbiota—to provide symptomatic relief, reduce recurrence rates, and improve outcomes in conditions like uncomplicated diverticular disease and IBD. Clinical studies have shown this combination to be more effective than either agent alone for symptom resolution and prevention of recurrence in these indications[1][2][3][4][7].
