The combination of mesalamine and metformin is being investigated as a potential treatment for ulcerative colitis (UC). This combination therapy involves administering both drugs together to achieve enhanced therapeutic effects compared to mesalamine alone. Mesalamine (also known as 5-aminosalicylic acid or 5-ASA) is an established treatment for mild to moderate ulcerative colitis, working as an anti-inflammatory agent in the colon[5]. Metformin, primarily known as an oral anti-diabetic medication, has shown promising anti-inflammatory properties that may complement mesalamine's effects in UC treatment[1][2]. Recent clinical trials have investigated this combination therapy: a randomized, controlled, double-blinded clinical trial included 60 participants with mild to moderate UC. The results showed that the metformin plus mesalamine group demonstrated significant decreases in inflammatory markers (including serum IL-6, zonulin, TNF-α, and S1P), reduced gene expression of ICAM-1 and STAT-3, increased colonic ZO-1 (a marker of intestinal barrier integrity), and significant decreases in pain scores (NRS) and partial Mayo score index[1]. The combination therapy appears to be well-tolerated, with no reported significant drug interactions between mesalamine and metformin[4].
