Overview

**Mivebresib + ruxolitinib** is an investigational oral combination therapy for myelofibrosis composed of mivebresib, a bromodomain and extraterminal domain (BET) inhibitor, and ruxolitinib, a Janus kinase (JAK) 1/2 inhibitor. Mivebresib suppresses BET protein-associated transcription of genes involved in inflammation, fibrosis, and cell proliferation, while ruxolitinib inhibits JAK1 and JAK2, downregulating cytokine signaling and reducing pathogenic inflammation. This combination aims to address two central pathways in myelofibrosis pathogenesis: inflammatory cytokine production and aberrant cell growth. Mivebresib + ruxolitinib is developed primarily for the first-line treatment of myelofibrosis in JAK inhibitor–naïve patients. The combination is based on findings that pair BET and JAK inhibition offers improved clinical benefits (spleen volume reduction, symptom improvement, better bone marrow morphology, and anemia response) versus JAK inhibitor monotherapy, as demonstrated by similar combinations such as pelabresib + ruxolitinib[3][5][6].
