MRI-1867 is a peripherally-restricted, dual-action hybrid small molecule being developed by Scopus BioPharma for the treatment of systemic sclerosis (SSc). It acts as an inverse agonist of the cannabinoid receptor 1 (CB1R) and an inhibitor of inducible nitric oxide synthase (iNOS). By targeting these two pathways simultaneously, MRI-1867 aims to address the complex pathophysiology of fibrosis and inflammation in SSc without crossing the blood-brain barrier, thereby avoiding central nervous system side effects typically associated with CB1 receptor modulation. Preclinical studies have demonstrated its efficacy in reducing fibrosis in the liver and lungs, with favorable oral bioavailability and pharmacokinetic properties supporting once-daily dosing. The drug was licensed from the National Institutes of Health (NIH) and developed in collaboration with the National Institute on Alcohol Abuse and Alcoholism (NIAAA).
