Obinutuzumab + mycophenolate mofetil + prednisone is a combination regimen under phase III investigation for the treatment of class III and IV lupus nephritis (LN), a serious manifestation of systemic lupus erythematosus. Obinutuzumab is a humanized, glycoengineered type II anti-CD20 monoclonal antibody that targets CD20 on B cells, leading to B cell depletion and immunosuppression. Mycophenolate mofetil is an oral immunosuppressive agent that inhibits inosine monophosphate dehydrogenase, a key enzyme in purine synthesis, thereby suppressing lymphocyte proliferation. Prednisone is a synthetic glucocorticoid used for its anti-inflammatory and immunosuppressive effects. This combination aims to achieve improved renal responses and disease control by synergistically targeting B cells, modulating immune activity, and reducing inflammation. The regimen is being studied primarily in adults with active lupus nephritis. Development is led by Roche, with clinical trials ongoing for LN and systemic lupus erythematosus[1][3][6][8][10].
