Ocrelizumab is a recombinant humanized monoclonal IgG1 antibody that selectively targets CD20-expressing B cells. CD20 is a cell surface antigen found on pre-B cells, mature and memory B cells, but not on lymphoid stem cells or plasma cells. By binding to CD20, ocrelizumab depletes these B cells through mechanisms including antibody-dependent cellular phagocytosis (ADCP), antibody-dependent cellular cytotoxicity (ADCC), and complement-dependent cytotoxicity (CDC). The drug is primarily indicated for the treatment of adults with multiple sclerosis, including relapsing forms (clinically isolated syndrome, relapsing-remitting disease, active secondary progressive disease) and primary progressive multiple sclerosis. It was developed by Genentech/Roche and first approved in 2017 under the brand name Ocrevus[1][2][5][6][7].
