Overview

Ofatumumab + daratumumab is an investigational combination therapy consisting of two monoclonal antibodies, each with distinct targets and mechanisms. Ofatumumab is a fully human anti-CD20 monoclonal antibody that binds to the small and large extracellular loops of the CD20 molecule on B lymphocytes, leading to B-cell depletion through complement-dependent cytotoxicity (CDC) and antibody-dependent cellular cytotoxicity (ADCC). It is primarily used in chronic lymphocytic leukemia (CLL) and other B-cell malignancies[6]. Daratumumab is a human IgG1κ monoclonal antibody targeting CD38, a glycoprotein highly expressed on multiple myeloma cells. It induces cell death via direct apoptosis, CDC, ADCC, antibody-dependent cellular phagocytosis (ADCP), and modulation of immune suppression[1][3][5]. The combination has not been approved as a fixed regimen but may be explored in research or off-label settings for hematologic malignancies where both CD20+ and CD38+ cells are implicated.
