Pelareorep (Reolysin) is an investigational, intravenously delivered, non-enveloped, double-stranded RNA (dsRNA) oncolytic reovirus (Type 3 Dearing strain) developed by Oncolytics Biotech. It is designed to selectively replicate in cancer cells with activated RAS signaling pathways, which are prevalent in pancreatic adenocarcinoma. The viral replication leads to direct tumor cell lysis and the release of tumor-associated antigens, converting immunologically "cold" tumors into "hot" tumors by stimulating innate and adaptive immune responses. In the treatment of pancreatic adenocarcinoma, pelareorep is administered in combination with various chemotherapy regimens—such as gemcitabine, irinotecan, or 5-fluorouracil/leucovorin—to synergistically enhance anti-tumor activity and overcome immune evasion. This combination approach has received FDA Fast Track designation for metastatic pancreatic ductal adenocarcinoma (mPDAC).
